Emergency department and trauma and orthopaedic staff at the Great Western Hospital started using the adapted bottle, collaborating with staff from NHS Trusts across the South West.

The surgeon specialised in children, but also carried out emergency orthopaedic procedures on adults, the trust's chief executive Roland Sinker said.

A woman working at an orthopaedic shop on the road where the incident took place told the BBC that half a dozen people came running into the shop.

"The areas that have been a particular challenge are endoscopy, where we've lost capacity, and planned orthopaedic operations," said Paul Mears, chief executive of the health board.

More surgical hubs will be created to focus on common, less complex procedures, such as cataract surgeries and some orthopaedic work.
