An investigation is under way and a probable cause of the crash will be determined in the NTSB's final report in 12 to 24 months' time.

A preliminary report from the National Transportation Safety Board, or NTSB, found that two electrical blackouts disabled equipment ahead of the incident, and noted that the ship lost power twice in the 10 hours leading up to the crash.

Peter Gautier, in a July letter to the NTSB, wrote that it had been “dilgently working” on a notice of a proposed rule making with “all due haste” on safety management systems.

The NTSB’s final report on the 2022 crash, which was released Thursday, said the probable cause was inadequate inspections by the pilot and maintenance personnel, resulting in an eventual loosening of the hardware and subsequent loss of helicopter control.

That documentation does exist and Boeing has provided it to the NTSB, the safety agency charged with investigating the incident, The Seattle Times has confirmed.
